Assists parents in tracking their child’s growth and development
Follow Along Provides:
Periodic developmental tracking through a series of age appropriate questionnaires (ASQs) sent every 4-6 months up to 36 months of age.
Age appropriate Activity Sheets with a variety of new things to try with your child in support of their ongoing learning and development.
Parent support with additional parent education and child development information, as needed.
Assistance and referral support in identifying and accessing needed services.
